• application form
• original and 2 copies of the academic credentials received at the previous education institution
• original and 2 copies of the diploma supplement
• copy of the birth certificate
• medical certificate certified by the official health authorities of the foreign resident’s home country and issued not later than two months before coming to Ukraine
• 2 copies of the passport or a document certifying the identity of the person without citizenship translated into Ukrainian language and notarized
• valid medical insurance policy
• 6 color photos, with dimensions of 3 centimeters by 4 centimeters
• a copy of Ukrainian foreigner license (if applicable)
• an envelope
• duo-tang file folder

Documents specified in subparagraphs from 2 to 6 of this paragraph have to be translated into Ukranian in Ukraine, the translation have to be notarized.

Documents must be notarized in the country of their issuance in a way that is officially used in this country for such certification, and legalized by the relevant overseas diplomatic institution of Ukraine, unless otherwise provided by international agreements of Ukraine.

Documents on education of foreigners admitted issued by the institutions of other countries undergo recognition in Ukraine in accordance with the legislation.

If the authenticity of academic credentials received by a foreigner at the previous education institution or level obtained is not confirmed, such a foreigner is sent down.
